The formal translation is

Seanmháthair.

or

Máthair mhór

We used say Seanmháthair (Shan waw hir) for Grandmother
and Seanathair (Shan ah hir) for Grandfather.
My translations may be basic, and you should maybe wait for corroboration from others.
