A chairde,
I'm looking to change my name from the Anglicised Kiernan to Mac Thighearnain.

My question is - is it pronounced "Mac-tear-nan" or "Mac-keer-nan" ?

That's an older spelling. The modern spelling is Mac Thiarnáin, though many families do retain the old spellings as often happens in any language when names have a long history.

Both would be pronounced with a k rather than a t, as th is a simple h sound and runs together with the c of mac to give a k sound. (It's really "muck HEER-na(w)n" but sounds like "muck KEER-na(w)n".)
